make plot functions robust to reduced number of change classes (fix issue 39)

The reason for the bug (issue 39) was that the plot functions only worked if all LULC change classes exist, but this was not the case for the Heidelberg area at this specific date range with the new LULC utility version. It's good that you reported this issue, as this should happen often. Should be fixed now.
